Your First Appointment

When you come to see Dr Guzman for your first appointment, please bring the following with you:

• A referral letter from your doctor

• Any previous medical imaging related to your condition such as x-rays, CT or MRI scans

• Your Medicare card

• Your private insurance details (if applicable)

• Workers compensation or third party details (if applicable) and a letter of approval for the consultation from the insurance company

During your first appointment with us, we want to make sure that you get the best treatment possible, which is why we need some information from you. Please make sure you have the following details on hand:

• Any medical conditions you have been diagnosed with in the past

• The names of medications you're currently taking.

• Any allergies you have

• Details of previous operations

• Your GP's name and contact details

• Patient Registration Form

• Medical History Form

• Medications and Allergy Form

• Privacy Act
